About the event

 Advanced care planning isn't just for someone that approaching the later years of their life or facing a life ending diagnosis. It is for everyone. We all die. It is an unfortunate fact of living.

Does your family know what your wishes are regarding death and dying? Do you have something laid out for those that have to speak for you when you cannot? How do you even begin to start the conversation?

Session #1 of Advance Care Planning is about how to assess what you want your healthcare plan to look like if you can't speak for yourself and how to get the conversation started, whether it's with yourself, with your physician, or with your family
